Dear Parents/Carers,
Re: Pupil Medical Information and Care Plans
As we begin using Medical Tracker in school, it is crucial that we have the most up-to-date information regarding any medical conditions of your children. Next week, parents/carers will receive notifications from Medical Tracker asking you to confirm the details of your child’s care plan. This is for those children with long term chronic conditions, such as asthma, sickle cell or severe allergies. Parents/carers will also receive a notification for those children administered any medication during the school day.
How this works:
Parents will receive notification via email with a four-digit code to log-in to the Parent Approval hub. From here you will be able to approve, request changes/review or cancel the care plan or prescribed medication.
Any requested changes will be made and a follow up approval request will then be sent for parents/carers to approve these new changes. See an example guide below.
Notifications will be sent out to parents/carers on Monday 7th October 2024. After this date, if you think you should have received a notification about a care plan or medication and did not, please ensure your child’s medical needs/conditions are up-to-date via Sims Parent and contact the school office. A member of the office team will be able to assist you.
