Dear Parents/Carers,

Please see the attached flu vaccination letter (with a link for consent to register your child) from the NHS regarding the all year group (Reception – Year 6) Flu vaccination Session on Wednesday 22^nd October 2025.

Parents/Carers are reminded to select the correct full school name (Kelvin Grove Primary School) and that only one parent needs to complete the consent form per child to avoid duplication. The e-consent form will be returned directly to the immunisation team. The forms will be triaged, and any queries will be answered by nurses. The school office does not receive any forms completed.

Please complete the online form to give consent and register your child if you would like them to receive the flu vaccination. If you do not want your child to have the flu vaccination, you should still complete the form to indicate that you do not give consent, which will prevent you from receiving NHS reminder messages.
