Dear Parents/Carers,

Please see the attached flu vaccination letter sent out previously from the NHS regarding the all year group (Reception – Year 6) Flu vaccinations and what to expect.

We have been given a catch up session date for those who missed the original session in October.
Our catch up session is Monday 24^th November 2025

Parents/Carers are reminded to select the correct full school name (Kelvin Grove Primary School) and that only one parent needs to complete the consent form per child to avoid duplication. The e-consent form will be returned directly to the immunisation team. The forms will be triaged, and any queries will be answered by nurses. The school office does not receive any forms completed.

Please complete the online form to give consent and register your child if you would like them to receive the flu vaccination. If you do not want your child to have the flu vaccination, you should still complete the form to indicate that you do not give consent, which will prevent you from receiving NHS reminder messages.
