Please see the letter below regarding the Lewisham and Greenwich NHS Trust (LGT) School-Entry Health and Wellbeing Questionnaire (HWQ).
The Health and Wellbeing Questionnaire (HWQ) is a valuable tool in identifying and addressing the health concerns and requirements of students within our educational community.
By parents completing this questionnaire, Lewisham and Greenwich NHS Trust can gain a comprehensive understanding of the health needs present within your school, which in turn enables us to:
· Provide direct care to children where any health needs are identified
· Develop bespoke health education sessions for pupils at Kelvin Grove
